This form is to be completed by any layperson seeking approval to lead a retreat or event within the Diocese of Maitland-Newcastle.

Before leading any retreat or event in the Diocese, lay leaders are required to obtain formal approval prior to the scheduled activity.
It is recommended that all documentation be submitted 4 weeks before your event date.
